Submitting an Order

If you are ready to move forward with scheduling a patient, select the appropriate order form below and either fill it out digitally, or print, complete and return to us via email at pcc@cerescan.com or fax (833) 299-2501. Once received, our PCCs will do the rest. If your patient decides to use his/her insurance plan to help cover the cost of imaging, CereScan may need clinical notes (patient chart history) from you. A CereScan team member will contact your office if these records are required.

Receiving the Final Report

After the patient has imaged and the radiologist has finalized his/her findings and impressions, our mid-level clinician will prepare the results and send copies to you and the patient. You may request to receive these results by encrypted email, fax, and/or mail.

Reviewing the report and follow-up questions

Our mid-level clinician will encourage the patient to schedule a follow up office appointment with you to go over the radiologist’s findings. If you have questions about the imaging results prior to the patient’s follow up appointment, please contact molson@cerescan.com to schedule a meeting (we know your time is valuable so we like to keep these short). CereScan also offers one-on-one consultations between our clinician and the patient.
